How This Site Works

Unlike typical information collection techniques, Feedback STAT is designed for your convenience. When invited to participate in a survey, you will be provided with a log in and password and asked to come to this web site. You decide when you would like to complete the survey and from where. All you need is access to the Internet. However, don't wait too long - surveys are not left 'open' forever.

Once you reach the site, simply click the button on the left labeled "Log In." You will be prompted for the login and password. The system will determine which survey you are participating in based on the login and password you provide. As soon as the survey appears on your screen, you will be given an estimate of how long the process should take. We make a concerted effort to make surveys brief and easy to understand. If you ever run into problems or need assistance, please contact us. Our phone number and e-mail address appears at the bottom of each page.

If the survey sponsor is offering a gift certificate in return for your participation, information on the certificate will appear at the end of the survey.
